Job Description Job Title: Family / Internal / Geriatric
Medicine Physician Location: Olivia / Wabasso / Franklin, MN Shift:
Full-time position Pay: $175 - $250 per hour Summary: We are
looking for a Physician to serve patients in Long-Term Care (LTC)
and Transitional Care Units (TCUs) across Olivia, Wabasso, and
Franklin, MN . This position requires the provider to float between
these locations to offer exceptional care to residents in
post-acute and long-term care settings. Responsibilities : Conduct
comprehensive patient assessments and ongoing medical evaluations.
Create and manage personalized treatment plans based on patient
health needs. Diagnose and treat acute and chronic conditions ,
providing timely medical interventions. Prescribe medications,
order diagnostic tests , and modify treatments as needed. Work
closely with nurses, therapists, and social workers to coordinate
comprehensive care. Requirements: MD or DO degree from an
accredited medical school. Board certification or eligibility in
Internal Medicine, Family Medicine, or Geriatrics preferred. Valid
medical license to practice in Minnesota. Previous experience in
long-term care, transitional care, or geriatrics is a plus.
