Home Health Overview: The Licensed Practical Nurse II is
responsible for delivering direct patient care under the guidance
of registered nurses or licensed prescribers. This role prioritizes
patient confidentiality and involves collaboration with various
departments. Candidates must hold a valid LPN license and Basic
Life Support certification, and the position entails physical tasks
such as lifting and assisting patients. The role underscores

role employs the nursing process to deliver patient care under the
supervision of individuals licensed by a State regulatory board to
prescribe medications and treatments, or under the direction of a
Registered Professional Nurse (RN). The LPN maintains
accountability for the care provided to assigned patients and
coordinates with other departments to ensure comprehensive patient
care. Upholding patient rights and confidentiality is essential,

service standards. Position Details: Qualifications: Education:
High school diploma or equivalent (GED) Licensure: Completion of an
accredited practical nursing program; equivalent educational
experience may be considered. The candidate must obtain and
maintain a current LPN license in the state where nursing duties
are performed and comply with all State Board of Nursing
regulations. Physical Requirements: The role requires standing or
walking for most of the shift, significant bending and stooping, as
well as lifting and turning patients with assistance. Ability to
push, pull, or grip up to 50 pounds is necessary. Experience: Open
to both new and experienced LPNs. Certifications: Basic Life
Support certification from the American Heart Association or
completion of the course within 30 days of hire. LPN-IV
Certification must be completed prior to or within 3 months of hire
(with a range from upon hire to within 1 year). Additional
certifications may be required based on the specific position or
unit, including but not limited to: ACLS (Advanced Cardiac Life
Support) PALS (Pediatric Advanced Life Support) PEARS (Pediatric
Assessment Recognition and Stabilization) S.T.A.B.L.E. (Sugar,
Temperature, Airway, Blood Pressure, Lab Work, And Emotional
Support) Other unit-specific certifications as necessary. Why Join
