Region 5 Systems is looking for Case Manager to join our team!
This is a remote-based position located in Lincoln, Nebraska.
Employees work from their residence (telework) and perform most
client-related field work in person within the communities served
by Region 5 Systems (southeast Nebraska). Some duties, including
required meetings and trainings, must be completed at the principal

You’ll Do Our Case Managers (known as “ Professional Partners ”)
help set the stage for success by supporting consumers in our
Family & Youth Investment program : Provide wraparound case
management services to youth and young adults (under age 26) with
serious emotional disturbances (SED) in southeast Nebraska. Support
youth who are at risk of out-of-home placement, school disruption,
or involvement with the juvenile justice or child welfare systems .
Work with participants and their families to develop
individualized, strength-based plans that build on natural supports
within their lives. Coordinate services with schools, community
providers, and support systems to help youth succeed at home, in
school, and in the community. Support youth who are transitioning
from the youth behavioral health system into adult services .
Maintain a manageable caseload (maximum of 15 participants) to
ensure meaningful support and engagement. Document services and
progress according to program guidelines. See job description for
full range of responsibilities. What You’ll Have Education &
Experience: Bachelor’s degree in human services or related field.
Recent and soon-to-be graduates are encouraged to apply! Applicants
expected to graduate within 3 months will be considered equivalent
to candidates who have already completed their degree. HR will
verify anticipated graduation following job offer. Extensive
experience (3 or more years) in related human services field may be
substituted for degree. Preferred Experience: 2 years working with
persons with serious emotional disturbances (SED) and/or 2 years’
experience in a behavioral health direct service position will make
you stand out. Remote Work Skills: Ability to work effectively in a
remote environment, including proficiency with video conferencing
tools (e.g., Teams, Zoom), electronic health records, shared
documentation, and network drives. Core Skills: Strong
communication, ability to facilitate team meetings, write concise
progress notes, prioritize multiple tasks, and coordinate with
external agencies supporting program participants. Other
