Job Description Job Description Description: The MDS Coordinator/ADON is responsible for the coordination, accuracy, and completion of the Minimum Data Set (MDS) assessments and care planning process in accordance with federal and state regulations. This role also assists the Director of Nursing with the overall management of the Nursing Department, including supervision of clinical staff, ensuring quality of care, staff education, and regulatory compliance. This position complies with department and organization-wide policies, long-term care facility requirements, safety, and infection control, and all federal and state of Nebraska regulations related to the performance of Heritage of Webster County operations and requirements of the compliance program. Requirements: ESSENTIAL DUTIES AND RESPONSIBILITIES Coordinates and manages the MDS assessment and care planning process. Ensures timely and accurate completion and submission of MDS assessments (OBRA and PPS). Monitors and maintains compliance with RAI guidelines and state/federal regulations. Collaborates with interdisciplinary team for care planning and MDS documentation. Analyzes data from MDS for care planning, quality measures, and reimbursement optimization. Reviews and updates care plans based on resident condition changes and MDS findings. Trains and supports nursing staff in accurate documentation and MDS processes. Audits MDS and documentation regularly to ensure accuracy and compliance. Works with billing and finance teams to ensure correct reimbursement under PDPM. Assists the DON in overseeing daily nursing operations and ensuring quality resident care. Supports the development, implementation, and evaluation of nursing policies and procedures. Participates in staffing, scheduling, and supervision of nursing staff. Conducts rounds to assess resident care, staff performance, and facility environment. Supports QAPI initiatives and infection prevention efforts. Participates in investigations and resolution of resident/family/staff concerns. Coordinates and delivers in-service education and staff development. Ensures compliance with all applicable federal, state, and local regulations. Performs other duties as assigned. Utilizes proper body mechanics, transfer/lifting techniques and appropriate equipment to minimize fall risk to resident and injury to self. EDUCATION AND/OR EXPERIENCE Minimum of 2 years’ experience in long-term care or skilled nursing. Prior MDS Coordinator experience preferred. Leadership or management experience in a healthcare setting preferred. CERTIFICATES, LICENSES, REGISTRATION Current Registered Nurse/Licensed Practical Nurse License from the State of Nebraska Current Basic Life Support (BLS) certification required.
