Job Description:
Prefer a Social Services Coordinator but will consider a Social
Services Designee Social Services Coordinator: POSITION SUMMARY:
Identifies personal and medically related social service needs of
the resident and assures that the individual needs are met by
coordinating services with appropriate disciplines. Makes
appointments and provides transportation to meet the medical and
personal needs of the resident. Documents assessments, care plans
and social service interventions per facility policy as
appropriate. REPORTS TO: Director of Nursing, LTC POSITIONS
SUPERVISED: None QUALIFICATIONS: • Qualified Social Worker
Preferred. MSW or BA with 1 year experience or Associates degree
with 1 year experience or degree in social/behavioral science with
1 yr experience OR successfully completed 36 hour social services
designee training approved by the state of Nebraska. • Heart Saver
CPR Certification • Attend 36 hours of training with DHHS to obtain
Social Services Designee training to care for residents in LTC
setting • Must have and maintain current driver’s license to drive
with acceptable driving record per facility standards • SHIPP
trained or must take training within 6 months of hire. • Must have
current CNA certification or must obtain within 9 months of
employment. • Must have current driver’s license and acceptable MVR
per JCH&L auto insurance. GENERAL REQUIREMENTS: To perform this

Services Designee: POSITION SUMMARY: Identifies personal and

also coordinates PIP teams, does QA audits, and attends and
participates in weekly care plan meetings. At times, SSD may be
pulled to work directly on the floor to ensure adequate care for
the residents. This is primarily a Monday-Friday position, but some
weekends or holidays may be assigned as needed. REPORTS TO:
Director of Nursing POSITIONS SUPERVISED: None QUALIFICATIONS: •
High school diploma or GED • Attend 36 hr training with DHHS to
obtain SS Designee certificate to care for residents in a LTC
setting. • Heart Saver CPR Certification • Must have and maintain

insurance, per facility standards. Accepted by JCH&L auto
insurance. • Must have current Nebraska CNA certification or obtain
