Job Description:
JOB SUMMARY: · The staff Pharmacist is responsible for providing
medication therapy and management services in collaboration with
other providers to assure that drug therapy is safe, effective,
efficient and cost-effective · Pharmacy services provided include
interdisciplinary patient care rounds, medication reconciliation,
medication therapy monitoring, medication orders management,
patient education, drug information, drug dosing, and various other
activities · Performs both clinical and distributive tasks
necessary to ensure that patients’ medication therapy is safe,
effective, timely and efficient · May be asked to provide training
and education to PharmD students through practical and didactic
experiences · Act independently within the scope of their license
and areas of expertise with the help of general policies,
procedures and practice guidelines with support from the Director
of Pharmacy · Ensures that patient care is maintained through
compliance with the department's policies, rules and regulations
promulgated by the Board of Pharmacy, and other applicable
regulatory bodies · Acts as Antimicrobial Stewardship Pharmacist as
assigned PREVIOUS EXPERIENCE NEEDED: · Staff - None TRAINING &
EDUCATION NEEDED: · Graduate from an accredited college of Pharmacy
· Currently licensed as a pharmacist in the State of Nebraska or
ability to obtain it within 3 months of hire FUNCTIONAL
REQUIREMENTS: · Able to communicate in the English language and
make assessments by speaking, hearing, seeing, feeling, reading and
writing · Works primarily in hospital setting, including
clinical/patient care areas and office or meeting areas ·
Considerable walking, sitting, or standing for long periods of time
· Mental effort required to assess, plan and make recommended
decisions in complex environment · Potential to be exposed to
blood/body fluids, including infectious diseases · Must be able to
maneuver throughout halls, stairways and patient rooms in response
to emergencies · Interpretation of environmental input requires
visual and auditory skills · In the event there is a need to
evacuate the building, lifting will be required to transport
patients to safety · Significant amount of keyboarding and writing
is required · Wearing of personal protective equipment (PPE) such
as gloves, masks, gowns and goggles may be required · Fine motor
skills · Long periods of intense concentration PHYSICAL

Work Practice 1. Order review and entry 2. Dispenses, and compounds
medication based on provider orders 3. Consults with physicians
regarding medications of pediatric, adolescent, adult, or geriatric
patients as appropriate 4. Checks medications filled by Pharmacy
Tech/Pharmacist for accuracy 5. Completes admission and discharge
medication reconciliations as needed 6. Provides educational
instruction to patients, nurses, providers, and public as needed 7.
Assists in maintenance of inventory and purchasing 8. Participation
on committees 9. Completes training to compound Sterile, Hazardous,
and Non-Sterile medications 10. Follows department and hospital
policies and procedures 11. Knowledgeable of pharmaceutical waste
12. Attends Code situations (Cardio-Pulmonary arrests) and Trauma I
situations 13. Proficient in all functions of pharmacy computer
system (Paragon Pharmacy) 14. Proficient in all pharmacy programs
(i.e. Pyxis, Pyxis Logistics, PharmacyKeeper, Sentri7, Control
Check, DoseMeRx, QS/1) 15. Assists with pharmacy students or job
shadows 16. Keeps accurate records relative to State and Federal
Regulations 17. Responsible for function of department in absence
of Director 18. See Individual department

duties 19. Demonstrates appropriate competencies in the department
with initial orientation competency due within 90 days of hire and
annual competencies to be completed by September 30th 20. Maintains
competency through completing education assigned on CCH learning
management systems as well as mandatory in person education 21.
Complies with Attendance Management Policy in regards to attendance
and timeliness, understanding the expectation of maintaining an
absence percentage of 3% or less 22. Accepts other duties within
